Virus hepatitis C genotip 1b sebagai faktor risiko utama yang terkait dengan karsinoma hepatoseluler pada pasien dengan sirosis: studi kohort prospektif tujuh belas tahun.
Imad Analis. Hepatocellular carcinoma (HCC) adalah penyebab kematian yang paling sering pada pasien dengan hepatitis C virus (HCV) -induced cirrhosis. Meskipun sejumlah studi di populasi yang berbeda di seluruh dunia menunjukkan hubungan antara HCV genotipe 1 dan risiko kanker hati, belum ada konsensus yang muncul mengenai hal ini, yang masih kontroversial.
Dalam upaya untuk memperjelas masalah ini, sebuah studi prospektif dari 163 pasien HCV-positif berturut-turut dengan sirosis, yang terdaftar antara Januari 1989 dan Desember 1990, dilakukan. Kejadian HCC terdeteksi oleh surveilans ultrasound setiap 6 bulan. Prediktor independen dari HCC dinilai dengan analisis regresi Cox.
Setelah rata-rata tindak lanjut 10,7 tahun, 44 [4,26 / 100 / tahun, interval kepercayaan (CI) = 3,11-5,68 / 100 / tahun] dari 104 pasien yang terinfeksi dengan genotipe 1b mengembangkan HCC versus 10 (1,69 / 100 / tahun, CI = 0,82-3,09 / 100 / tahun) dari 52 pasien yang terinfeksi dengan genotipe 2a / c (P = 0,0001).
Analisis multivariat menunjukkan bahwa HCV genotipe 1b secara independen terkait dengan pengembangan HCC [rasio hazard (HR) = 3,02, 95% CI = 1,40-6,53]. Prediktor lain dari HCC adalah varises esofagus (HR = 2,15, 95% CI = 1,03-4,47), jenis kelamin laki-laki (HR = 2,12, 95% CI = 1,10-4,11), dan usia di atas 60 tahun (HR = 5,96, 95% CI = 1,23-28,8).
